Dabigatran etexilate is a small molecule prodrug that does not show any pharmacological activity. Following oral administration, dabigatran etexilate is rapidly absorbed and converted to dabigatran by esterase-catalyzed hydrolysis in plasma and liver. Dabigatran is a potent, competitive and reversible direct inhibitor of thrombin and is the main active ingredient in plasma. Since thrombin (serine protease) allows the conversion of fibrinogen to fibrin in the coagulation cascade, its inhibition prevents thrombus formation. Dabigatran inhibits free thrombin, fibrin-bound thrombin, and thrombin-induced platelet aggregation.
